Accident Report Investigation Form
If you are injured or involved in a near miss incident you must inform your on-site supervisor immediately and you must complete an Accident/incident Report Investigation form (download below). Please DO NOT leave the site before notifying your supervisor and inform your Challenge Recruitment Ltd Consultant that you have been injured.
You must submit a Challenge Recruitment Ltd Accident/incident Report Investigation form WITHIN 24 HOURS of an accident/incident occuring. If you need to attend a doctor please call your Challenge Recruitment Ltd Consultant who will provide you with further information and instructions.
If you have medical expenses,or the doctor has given you time off work due to the injury, you will need to obtain a WorkCover certificate from the doctor and you must come into our office to fill out Workers Compensation claim forms. You must bring in your WorkCover certificates with you. If you choose not to submit a Workers Compensation claim, no time lost or medical expenses can be paid by Challenge Recruitment Ltd.
In the event the Doctor certifices you fit for suitable duties, inform your Challenge Recruitment Ltd Consultant who will arrange alternate duties for you either with the same employer or at another site (where ever possible).
